4. Patient Responsibilities
As a patient of DermCARE Practitioners, you agree to:

5. Appointment Policy
Scheduling
Appointments can be scheduled by calling (502) 361-3909 or texting 1-502-385-3191 during business hours. Text messaging is for appointment scheduling only and should not be used for medical advice or emergencies.
Cancellations and No-Shows
We require at least 24 hours’ notice for appointment cancellations or rescheduling. Failure to provide adequate notice or repeated no-shows may result in a cancellation fee or termination of the patient-practice relationship.
Late Arrivals
Patients arriving more than 15 minutes late for their scheduled appointment may need to be rescheduled to ensure quality care for all patients.
6. Payment Terms
Accepted Payment Methods
We accept cash, credit cards, debit cards, and other payment methods as posted in our office.
Payment Responsibility
Payment is due at the time of service unless prior arrangements have been made. You are responsible for understanding your insurance coverage and any out-of-pocket expenses.
Insurance
We work with many insurance providers. However, it is your responsibility to verify your insurance coverage, obtain necessary referrals or authorizations, and understand your benefits before your appointment. You are responsible for any services not covered by your insurance.
Cosmetic Services
Cosmetic procedures are typically not covered by insurance and payment is required at the time of service.
